With the development of AO/OTA classification of ankle and foot fractures, Lauge Hansen classification of ankle, Hawkins Canale classification of talus fractures, Sanders classification of calcaneal fractures, Myerson classification of tarsometatarsal injuries, and Olerud Molander ankle fracture functional scoring system, AOFAS ankle and hind foot scoring system, Maryland foot functional scoring system With the establishment of the system, the curative effect of ankle bone injury has been greatly improved. However, the principles of treatment based on these types are still controversial, and more papers on the mechanism, biomechanics and treatment of ankle and foot trauma are published every year. There is no uniform standard for the treatment of ankle instability, especially for the chronic injury of the tibiofibular ligament syndesmosis. There are two theories about chronic ankle pain: (1) ankle instability; (2) proliferative soft tissue impingement in the tibiofibular syndesmosis, but the diagnosis is difficult. The indications for repair of medial and lateral injuries of the ankle are uncertain. The treatment principles of osteochondral injuries of the talus are also controversial.
Minimally invasive techniques have gradually been used to treat ankle loose bodies, cartilage injuries, synovitis and osteoarthritis. Microscopic ankle arthrodesis has been reported in many countries, but the surgical indications and techniques are not perfect, and there are many complications during and after the operation.
The end stage treatment of ankle arthritis is also the focus of discussion in ankle surgery. Arthroplasty or arthrodesis is controversial? With the design of various ankle prostheses and long-term clinical trials reporting, the status of arthrodesis as the "gold standard" for end-stage treatment of ankle arthritis has also been challenged.
Treatment of diabetic foot and ankle peripheral neuropathy is also a difficult point in ankle and foot surgery. The indication, timing and level of amputation become more stringent with the improvement of early diagnosis and treatment techniques and the development of vascular surgery.
Surgery and postoperative rehabilitation of various malformations of foot and ankle also have great room for improvement with the continuous improvement of people's quality of life requirements.

Foot and ankle surgery is a new subject in the world. In China, it is still in its infancy. The discipline construction and personnel training are not perfect. With the advent of the information age, international and domestic exchanges have become more convenient. It is both an opportunity and a challenge for the relatively backward development of domestic foot and ankle surgery.
